This is an official site of the Florida courts. An example of a standardized request for production from a third party can be found here. Select "General Public/Self-Help/Family Forms" and then scroll to "Discovery/Subpoena for Production of Documents from a Nonparty."

This is the site for the New Hampshire Bar
Association. An interesting New Hampshire Bar's ethics committee opinion can be accessed at this site. Select "Legal Links/Ethics Opinions and Practical Ethics Articles." Under 2008-09 Ethics Opinions, select Opinion "2008-04 Disclosure, Review and Use of Metadata in Electronic Materials." Essentially the opinion says that the sender of electronic materials must use reasonable care to avoid improper disclosure of confidential materials and the receiver has a duty not to search for, review, or use confidential material contained in metadata received from opposing counsel.
